To the Editor: Law et al. (May 22 issue)1 report that patients with idiopathic thrombocytopenic purpura who have poor responses to high-dose intravenous immune globulin are unlikely to have good or excellent responses to splenectomy. In our experience, however, the majority of patients with poor responses to intravenous immune globulin do respond well to splenectomy.

Our registry comprises 158 adults with idiopathic thrombocytopenic purpura (median age, 45 years) who were followed for a median of 3.4 years.
