To the Editor: Van der Wouw and Bax (Feb. 6 issue)1 reported on a woman with massive pulmonary embolism who died five hours after initiation of treatment with tissue plasminogen activator. Recently, a 51-year-old man was admitted to our cardiac care unit. Despite continuous treatment with low-molecular-weight heparin, deep-vein thrombosis had developed in both legs six weeks after surgery on his patella in another hospital. Transthoracic echocardiography showed marked dilatation of the right atrium and ventricle with a peak pulmonary-artery pressure of 60 mm Hg. The left atrium and ventricle were of normal size.
