To the Editor: In his review article on low-molecular-weight heparins (Sept. 4 issue),1 Dr. Weitz states that "preoperative dosing may be a problem in patients undergoing spinal anesthesia, although spinal cord bleeding is rare." The reference cited was a 1993 review by Bergqvist et al.,2 which noted a single case of spinal hematoma among the estimated several million patients who had received low-molecular-weight heparin while undergoing spinal or epidural anesthesia. The review represented only the European experience with low-molecular-weight heparins as prophylaxis against thrombosis, because no preparation of low-molecular-weight heparin had been approved for use in the United States at . . .
