To the Editor: Pecherstorfer (Nov. 13 issue)¹ describes a patient who presented with localized melanoma that was initially treated with surgery and adjuvant interferon therapy. Subsequently, regional in-transit melanoma and nodal failure occurred; the patient was turned down for limb perfusion, despite the development of extensive in-transit disease, before dying of distant metastases. Readers should not conclude from the clinical description that all patients with intermediate-thickness melanoma should receive adjuvant interferon treatment, nor should they conclude that perfusion should be withheld from all patients with in-transit melanoma because of the presence of synchronous regional nodal disease.
