A 58-year-old woman was admitted to the hospital because of chest pain. The night before admission, the patient awoke with crushing, nonpleuritic chest pain radiating down her left arm, with associated presyncope and diaphoresis. The pain was like cardiac chest pain that she had experienced previously. She had no dyspnea, fever, chills, or cough. She had long-standing hypertension and diabetes mellitus in association with corticosteroid therapy for idiopathic thrombocytopenic purpura, as well as a family history of premature coronary artery disease.
