To the Editor: Krum and colleagues (March 19 issue)1 make theclinically important observation that the orally active endothelinETA- and ETB-receptor antagonist bosentan causes a sustainedreduction of blood pressure in patients with essential hypertension.Although the study was carefully designed and performed, wehave some issues of concern related to the interpretation ofthe results.
First, the authors state that the reduction in blood pressurebrought about by bosentan suggests that endothelin-1 contributesto the elevation of blood pressure in patients with essentialhypertension, implying causation.
