To the Editor: Spinal epidural lipomatosis, or excess epidural fat, can compress nerve roots or the spinal cord. It occurs in patients with morbid obesity, hypothyroidism, and exogenous Cushing's syndrome1 but rarely in those with endogenous Cushing's syndrome.2,3,4 We describe a patient with spinal epidural lipomatosis who had the ectopic corticotropin syndrome.

The patient was a 36-year-old man who presented with severe back pain and weakness.
