To the Editor: Recently, the American College of Cardiology and the American Heart Association released an expert consensus document on the use of sildenafil (Viagra) in patients with cardiovascular disease.1 The recommendations focused on patients with coronary artery disease, but little is known about the effect of sildenafil on patients with other cardiac disorders.

We report on a 54-year-old man with hypertrophic cardiomyopathy. Echocardiography performed in 1994 showed no gradient at rest and only mild obstruction of the aortic outflow tract after inhalation of amyl nitrite.
