To the Editor: In their review of nephropathy in pa-tients with type 2 diabetes mellitus (Oct. 7 issue),¹ Ritz and Orth focus on treatment with antihypertensive drugs, mainly angiotensin-converting–enzyme inhibitors. However, other treatments can be effective, suggesting that there may be additional pathogenic mechanisms in diabetic nephropathy.

Cytokines, including tumor necrosis factor (TNF-), have been implicated in the development and progres-sion of diabetic nephropathy.² Pentoxifylline is a methylxanthine derivative with rheologic properties that is also a modulator of immune function, since it can decrease the serum TNF- level.
