To the Editor: In the retrospective study by Maron et al. (Feb. 10 issue)1 of patients with hypertrophic cardiomyopathy who received implantable cardioverter–defibrillators, the rate of appropriate discharge was quite high when the device was implanted in patients with hypertrophic cardiomyopathy after resuscitation from cardiac arrest. Clearly, some form of therapy is required for such patients.

However, the conclusion by Maron et al. that the use of an implantable cardioverter–defibrillator is warranted for the primary prevention of sudden death in high-risk patients with hypertrophic cardiomyopathy is not sufficiently supported by the available evidence.
