To the Editor: We are concerned about the definition of metastatic breast cancer used by Stadtmauer et al. (April 13 issue)1 in their report on high-dose chemotherapy with autologous stem-cell transplantation in women with metastatic breast cancer. By combining women with locally recurrent disease and women with distant metastatic spread, the authors may have introduced a considerable bias in the survival analyses. Women with local recurrences (i.e., cutaneous metastases) do better than women with distant metastases.2,3,4 Also, the survival data for women with locoregional axillary recurrences differ from those for women with distant metastases.5 Unfortunately, the authors do not indicate . . .
