Presentation of Case

A 59-year-old man was admitted to the hospital because of right-sided visual loss and oculomotor-nerve palsy.

The patient had had type 2 diabetes mellitus for 25 years; during most of those years he had taken insulin. Four years before admission, he had had an acute myocardial infarction, and more recently, chronic obstructive pulmonary disease, hypertension, and schizophrenia had been diagnosed.

One week before admission, the patient was admitted to another hospital because of coughing and an inability to open his right eye. The temperature was 38.5°C. There was ptosis of the right upper eyelid, with evidence of oculomotor-nerve palsy. Laboratory . . . [Full Text of this Article]
