In 1988, a landmark study was reported on the use of adjunctivetreatment with dexamethasone in infants and children who hadbacterial meningitis.1 The rationale for this study was basedon studies involving animal models of bacterial meningitis, which demonstrated that the subarachnoid-space inflammatoryresponse was a major factor contributing to morbidity and mortalityamong patients with this disorder. This inflammatory responseis generated through local release by the central nervous systemof proinflammatory mediators such as interleukin-1 and tumornecrosis factor in response to lysis of meningeal pathogensinduced by antimicrobial agents. The consequences of this inflammatoryresponse were . . . [Full Text of this Article]
