A 76-year-old woman with a history of diabetes mellitus and myocardial infarction presented with abdominal pain. A plain radiograph (Panel A) showed an air and soft-tissue doughnut sign in the pelvis. A computed tomographic scan obtained subsequently (Panel B) showed the presence of multiple gas pockets around the urinary bladder. Pyuria with Klebsiella pneumoniae on culture and evidence of submucosal emphysema on cystoscopy confirmed the diagnosis of emphysematous cystitis. The patient was treated with antibiotics, and the clinical and radiographic findings resolved rapidly.
