Background The prevention of active tuberculosis through thetreatment of latent tuberculosis infection is a major elementof the national strategy for eliminating tuberculosis in theUnited States. Targeted treatment for persons who are at thehighest risk for reactivation tuberculosis will be needed toachieve this goal. A more precise assessment of the lifetimerisk of reactivation tuberculosis, usually estimated at 5 to10 percent, could help to identify patients who are at the highestrisk and motivate them to complete treatment. Currently, therate of completion of treatment is low.
Methods Published reports were reviewed to obtain estimatesof the risk of tuberculosis among persons with a positive tuberculinskin test. Using these data, I constructed a model to estimatethe lifetime risk of tuberculosis among persons with specificmedical conditions.
Conclusions Persons with these characteristics should be targetedfor intensive efforts to ensure full treatment of latent tuberculosis.Improved rates of completion of treatment among such personscould help to eliminate tuberculosis in the United States.
