Background Tuberculous meningitis kills or disables more thanhalf of those affected with the disease. Previous studies havebeen too small to determine whether adjunctive treatment withcorticosteroids can reduce the risk of disability or death amongadults with tuberculous meningitis, and the effect of coinfectionwith the human immunodeficiency virus (HIV) is unclear.
Methods We performed a randomized, double-blind, placebo-controlledtrial in Vietnam in patients over 14 years of age who had tuberculousmeningitis, with or without HIV infection, to determine whetheradjunctive treatment with dexamethasone reduced the risk ofdeath or severe disability after nine months of follow-up. Weconducted prespecified subgroup analyses and intention-to-treatanalyses.
Conclusions Adjunctive treatment with dexamethasone improvessurvival in patients over 14 years of age with tuberculous meningitisbut probably does not prevent severe disability.
