To the Editor: Almond et al. (April 14 issue)1 excluded an important variable in their assessment of hyponatremia in marathon runners. Excess fluid intake is clearly a contributor to low serum sodium concentrations,2 but numerous long-distance runners in the study population ingested high-sodium endurance gels. These popular supplements were offered by the race sponsors at mile 17 of the Boston Marathon or carried by the runners and ingested ad lib. Most of the gels contain 40 to 100 mg of sodium per packet of 32 to 41 g. The typical marathoner who completes the race in four to five hours . . . [Full Text of this Article]
