To the Editor: The study of subthalamic nucleus stimulation for the treatment of severe obsessive–compulsive disorder (OCD), reported by Mallet and colleagues (Nov. 13 issue),1 appears to be problematic for several reasons. First, on the basis of the clinical information given, such as age at onset, duration of disease, and prior treatment, we think the patients may not have been selected carefully enough. A major proportion of the study patients met the criteria for treatment-resistant OCD if anything,2 and thus might not have had an indication for neurosurgical treatment.
